Despite these requirements however, a large number of RCTs with features that defy the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all types. This could lead to false claims of pragmatism and the use of the term should be standardised. The creation of the PRECIS-2 tool, which provides an objective standard for assessing pragmatic features is a good initial step. Methods In a pragmatic trial it is the intention to inform clinical or policy decisions by demonstrating how an intervention would be incorporated into real-world routine care. This is distinct from explanation trials, which test hypotheses about the cause-effect connection in idealized situations. In this way, pragmatic trials could have less internal validity than explanatory studies and are more susceptible to biases in their design, analysis, and conduct. Conclusions In recent times, pragmatic trials are increasing in popularity in research because the value of real world evidence is increasingly recognized. They are clinical trials that are randomized that evaluate real-world alternatives to care instead of experimental treatments in development, they have populations of patients which are more closely resembling those treated in routine medical care, they utilize comparators which exist in routine practice (e.g. existing drugs) and rely on participant self-report of outcomes. This approach has the potential to overcome limitations of observational studies, such as the biases associated with reliance on volunteers and limited accessibility and coding flexibility in national registry systems. Pragmatic trials have other advantages, like the ability to draw on existing data sources and a greater probability of detecting meaningful distinctions from traditional trials. However, these trials could be prone to limitations that compromise their credibility and generalizability. Participation rates in some trials may be lower than anticipated due to the health-promoting effect, financial incentives, or competition from other research studies. Practical trials are often restricted by the necessity to enroll participants in a timely manner. Certain pragmatic trials lack controls to ensure that any observed differences aren&#39;t due to biases that occur during the trial. The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and that were published until 2022. They assessed pragmatism by using the PRECIS-2 tool that includes the eligibility criteria for domains and recruitment criteria, as well as flexibility in adherence to interventions, and follow-up. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or higher) in at least one of these domains.
